Early Education Funding - Forms and Documents

Privacy Notice

All providers who are registered to deliver early education funding are obliged to inform parents that information about children accessing early education funding at their setting will be given to the DfE (name, address and date of birth).

This is now known as a privacy notice (formerly a fair processing notice). The notice can be downloaded using the pdf below.

Parent Declaration Form

As part of the new Code of Practice providers and parents are required to complete either a Parental Declaration Form or a Parental Contract.

The Parental Declaration Form declares the child’s eligibility according to their date of birth as proven by a birth certificate (or other evidence).

The Parental Contract declares the child’s eligibility according to their date of birth as proven by a birth certificate (or other evidence), and sets out the hours and patterns of access to the free entitlement that the parent and provider have agreed to for a defined period (eg. termly/3 monthly).

Starter/Leaver Form

Providers need to complete a starter/leaver form for any funded children who start, leave or change their hours mid term.

The form is available below to download either as an excel spreadsheet or as a pdf.
